Transition
Individualized Education Program (IEP) transition planning is a crucial component of special education designed to assist students with disabilities as they move from school to post-school life. The transition planning process is initiated before the student’s 14th birthday. The main goal of IEP transition planning is to help students with disabilities successfully transition into adulthood by providing them with the necessary skills, support, and services. The transition plan is a formalized part of the student's IEP, and it focuses on various areas, including education, employment, and independent living.
